RULES OF THE ROAD
- NO STUDENTS MAY DRIVE THEMSELVES OR ANY OTHER STUDENTS TO AN EVENT/CONTEST PER EDUHSD GUIDELINES
- Please use current district forms below; old forms will not be accepted
- Forms must be submitted 10 (ten) days prior to event
- They may drive to Oak Ridge or Oak Ridge practice facility only
- If your athlete does not have paperwork turned in and approved then they will need to ride to and from with their parents
- Parents must be listed as “approved to drive” to carpool students from Oak Ridge to any event (with the exception of their own student)
- Staff members must also have their “Volunteer Personal Automobile Use Form” on file and approved before driving any student to an event, etc
- A student must be listed as having a “Student Alternate Transportation Form: Designation of Adult Driver” form on file to be able to ride with an approved driver (other than their own parent)
- If your athlete does not have paperwork turned in and approved then they will need to ride to and from contests with their parents
- If your athlete participates in more than one sport, list all sports on the ‘Designated Driver Form’ so you will not need to complete additional paperwork for another team
REQUIRED FORMS
- Must use current forms attached below, outdated forms will not be accepted STUDENT DESIGNATED DRIVER FORM (must be completed for all student athletes that may require alternate transportation by a district approved driver) VOLUNTEER PERSONAL AUTOMOBILE USE FORM (required to obtain district approved driver designation to transport student athletes other that your own to and from school related events)
- Attach copy of current/non-expired driver’s license for each district approved driver
- Attach copy of insurance policy DECLARATION PAGE and expiration for each vehicle/driver
****Must have minimum of $100k/$300k liability coverage to be approved
