ATHLETIC CLEARANCE & REGISTRATION
  3. Submit completed SPORTS PARTICIPATION HEALTH RECORD FORM.  The form must be completed on May 1, 2025, or after and submitted to the Athletic Office prior to try-outs. DO NOT UPLOAD your Sports Participation Health Record.  Please submit the original form to the Athletic Office.
  4. Verify all fines are cleared through the Finance Office – Don’t wait.
  5. Incoming Freshman should use 0000 for ORHS Student ID # until they receive their permanent #.
You must also complete the following if this applies to you:
    -Submit completed MEDICATION IN SCHOOL FORM if the student athlete is required to take medication during school activities.

FALL SPORTS- June 1, 2025
WINTER SPORTS- October 12, 2025
SPRING SPORTS- January 11, 2026
